New Patient Information

Dr. Yuko is currently accepting new patients 40 and under.

Appointments can be conducted in both English and Japanese.

Getting started is easy as 1, 2, 3:

  1. Please download and complete: New Patient Forms (click here).

  2. To submit the forms, you may:

    • Drop off the forms in person from 8AM - 1PM or 2PM-4PM

    • Mail your forms to our office

    • Fax your forms to 617-232-8086

    • We cannot accept forms over email due to the sensitive nature of your healthcare information.

  3. Once received by the office, the office will reach out to schedule the initial visit.

IMPORTANT (PLEASE READ):

  • Please submit a copy of your insurance card, front and back, with your forms. If you come into our office to drop off the forms, we can make a photocopy in the office.

  • Adults:

    • All adult new initial visits are via televisit to review your medical history, touch base on your personal preferences, and get to know you holistically. Annual physicals, sick visits, etc, are scheduled after the initial televisit.

  • Pediatric Patients:

    • For new children from Japan, please submit a copy of the Mama’s Book when submitting your forms.

    • Any new pediatric patients seen in the United States by another practice must provide the complete health record from their previous doctor’s office prior to scheduling the first visit, including all office notes and vaccination records. This is usually done by contacting your previous doctor’s office and requesting a “medical records release” to a new doctor’s office, and they will ask you to sign a form. Afterwards, they will send the records over to our office.

    • Pediatric patients 12 years old and under are seen in the office for the first visit.

    • Patients 13 and older will have their initial visits via televisit.

    • Please note that our office follows the CDC Vaccination Schedules.

We do not accept walk-ins. We can only accommodate scheduled patients in office.

  • Sick visits: please contact the office first thing in the morning in order to obtain a same-day visit. Televists are offered. We strive to fit in our sick patients within 24 to 48 business hours.

  • Medication refill requests: Non-controlled can take up to two to three business days, controlled substances can take up to five business days.

  • Insurance referral requests: may take up to three business days. When requesting an insurance referral please be sure to have the specialist full name, fax number and NPI number.

  • Administrative forms: such as school and camp forms may take up to seven business days.
