Your medical records include, but are not limited to, your medical history, test results, vaccinations, and notes from office visits. Yuko Family Medicine maintains your medical records securely through our electronic health records system (EHR) and can provide you with copies of your medical records.

Our office is not available for walk-in requests for medical records. If you need to collect records in person, please review this page and contact us as (617) 566-9856.

You can use Healow, our patient portal, to view your medical records online, where your information is available to download and print. Please click the green patient portal button on this page to access or enroll. Please call our office at (617) 566-9856 if you are having issues.

Send your medical records to another doctor

If you received care at Yuko Family Medicine and would like to send those notes directly to another doctor, please download and fill out the authorization form below.

Have another doctor send records to Yuko Family Medicine

If you would like medical records from another doctor sent to Yuko Family Medicine, please fill out the authorization form below. You may also fill out an authorization form from another doctor and have them send it to our office. Our fax number is (617) 232-8086.

Request a physical copy of your medical records

For copies of medical records sent directly to you, you will be charged a $15.00 base fee + $0.50 per page for the first 100 pages, then $0.25 per page after the first 100 pages according to HIPAA 45 CFR,164.524 and Massachusetts law (M.G.L. c. 111, § 70).

Requests relating to the Social Security Act and any federal or state financial needs-based benefit program will not be charged a fee.
